The Blue People of Kentucky: The Family Whose Skin Was Genuinely Blue for Generations
In 1820, a French orphan named Martin Fugate arrived at Troublesome Creek in eastern Kentucky and claimed a land grant on its banks. He married a local woman named Elizabeth Smith. She had red hair and skin described as pale as the mountain laurel that blooms around the creek hollows each spring. He had blue skin. Four of their seven children were born bright blue.
This is not a legend or an exaggeration. The Fugate family of eastern Kentucky produced genuinely blue-skinned people across multiple generations, and the condition persisted in the family for over 150 years, until a baby named Benjamin Stacy was born blue in 1975 and was among the last to show the full effect.
What Was Actually Happening
The condition is called methemoglobinemia. Understanding it requires a short detour into blood chemistry, but it is worth it. Normal red blood cells use hemoglobin to carry oxygen around the body. Hemoglobin gets its red color from iron atoms that bond with oxygen in a specific way. A key enzyme, cytochrome b5 reductase, keeps this system running correctly by continuously converting a faulty form of hemoglobin, methemoglobin, back into the functional kind.
The Fugates lacked sufficient amounts of this enzyme. Without it, methemoglobin accumulated in their blood instead of being recycled. Methemoglobin cannot carry oxygen efficiently and has a blue-brown color rather than a red one. When enough of it builds up in the bloodstream, it shows through pale skin as a distinct blue or purple tint.
The Fugate family carried the gene in a recessive form. Both parents need to carry it for a child to show the full effect. By extraordinary coincidence, Martin Fugate and Elizabeth Smith were both carriers, despite being strangers before they met. Four of their seven children inherited two copies and were born blue.
Why It Spread
Eastern Kentucky in the 19th century was genuinely isolated. Rail lines and paved roads did not reach Troublesome Creek until nearly a century after the Fugates arrived. The creek’s terrain made travel difficult in every direction.
When your world is a narrow mountain hollow with a small and stable population, genetic diversity is limited. The Fugate children intermarried with neighboring families, and some of those neighboring families also happened to carry the same recessive gene. The blue trait passed through the family tree repeatedly. One descendant, Dennis Stacy, described the situation this way: “It was hard to get out, so they intermarried. I’m kin to myself.”
Luna Fugate, a descendant who lived into her 80s, was described as one of the bluest of all the Fugates, a deep indigo blue across her whole face and body. She was healthy, worked, and raised a family. The methemoglobinemia the Fugates carried was a mild form, affecting appearance without causing the serious organ damage that higher concentrations of methemoglobin can produce. Their blood methemoglobin levels ran between 10 and 20 percent. Levels above 30 percent cause shortness of breath and neurological symptoms. Above 70 percent, the condition becomes fatal. The Fugates were blue, but not dangerously so.

The Doctor Who Found Them
In the early 1960s, a hematologist at the University of Kentucky named Madison Cawein heard rumors about blue people living in the hills near Hazard. He went looking.
At a heart clinic in Hazard, he came across a brother and sister, Patrick and Rachel Ritchie, with unmistakably blue skin. He mapped their family history, ruled out heart disease, and suspected methemoglobinemia. Testing confirmed it. Cawein then drove through the mountain hollows finding other blue Fugate descendants, administering a treatment called methylene blue by injection.
Methylene blue works as an electron carrier that helps restore the conversion of methemoglobin back to hemoglobin. The effect is rapid. Patrick Ritchie reportedly turned pink in front of Cawein within minutes of receiving the injection. Cawein taught the family to take it orally as a daily pill. The blue faded in those who took it.
The Ritchies had spent years being the subject of gossip. They were ashamed of their skin. The treatment changed their lives, though the gene remained in the family regardless of whether anyone showed its effects.
